Using Dragon Medical Outside of the Office
Many healthcare providers prefer (or need) to document their patient encounters outside of the office/clinic environment. Whether they’re catching up on patient charts or working remotely, clinicians regularly ask us about installing Dragon® on their laptops, dictating with their smartphones, routing files to transcriptionists, and more.
The version and edition of Dragon® that we’ll be referring to in this article is Dragon® Medical Practice Edition 2. When working with this application, you will always need a computer—whether that’s a laptop or desktop—to install it on. There is no current Dragon® Medical Practice Edition 2 application available for the smartphone. Rather, some of the tips we will be providing refer to third party applications and/or hardware.
Working From a Laptop
Many providers want to know if they can install Dragon® Medical on both a work computer and a home laptop. Yes, you can install Dragon® Medical Practice Edition 2 on up to four computers. If you get beyond four computers, uninstall Dragon® from one to free up another install, or consult with your Dragon® reseller. They may be able to get you more activations from Nuance directly. Aside from portability, there’s not a great deal of physical difference between using Dragon® dictation on a laptop and a desktop. You will have fewer USB ports, which may mean you need to unplug something to plug in your speech recognition microphone.
You will want to check your laptop’s system specs, to see if they differ from your clinic workstation. If your laptop has less computing power, you’ll probably notice it when using Dragon® Medical. If you want to improve this performance, consult with your IT department.
If you dictate directly into an EHR at the office, there’s a decent possibility that you won’t be able to at home. Because of the various layers involved in accessing EHRs, it’s likely that you will need to use the Dictation Box in Hidden mode to make your notes. This is definitely a workable solution, and we regularly train clinicians on how to do this successfully.
If your electronic health record software is installed locally on your laptop, along with Dragon® Medical speech recognition, you should be able to dictate directly into your patient charts. Caveat: If your EHR does not support Dragon® Medical Practice Edition 2 natively, you will still need to utilize the Dictation Box.
If your EHR is published to the World Wide Web, and can be accessed publicly, you should be able to use Dragon® Medical voice recognition directly in the application. You will need to ensure that the web browser has the Dragon® plug-in installed/enabled for full compatibility. Again, the same caveat applies: EHRs that don’t natively support Dragon® Medical require the Dictation Box.
Maybe you aren’t even making patient notes in an EMR. You might have a Microsoft Word template that you dictate into, or some other word processing software. Check the Dragon® Medical Practice Edition 2 Administrator Guide for supported applications.
Your EHR may be installed on your work computer, hosted on an application server, or delivered in some other method than locally. It’s very possible that you will need to remote into your main workstation, VPN into your network, or connect to your EHR through multiple virtualization layers.
In each of these cases, when using Dragon® Speech Recognition on your laptop, you will need the Dictation Box. You may also have to perform additional setup/configuration. Please note, there is no guarantee that Dragon® Medical Practice Edition 2 will work in every deployment. Refer to the administrator’s guide linked above for more details.

How to Renew Your Olympus Smartphone App Subscription
Why Does the App Need Renewal?
Technically speaking, the Olympus® Dictation app is free. You can download it to your Android or iOS smartphone and use it without paying a dime. You can even send dictations via email (in AMR or M4A formats, depending on your phone). But, in order to send dictations via the Olympus Dictation Portal, which supports encryption and records in either DSS or DS2 formats, you need to purchase an annual subscription.
If your transcriptionist has been receiving dictations from the Olympus Dictation Delivery Service, you are subscribed to the Olympus Dictation Portal. Today, we're going to talk about how to renew that subscription.
When Should I Renew?
Sixty (60) days before your license expires, you're going to receive an email telling you that it's time to renew. For most people, this is a five minute task—and we'll go over it in just a moment.
NOTE: Once you hit thirty (30) days from expiration, it is too late to renew your license.
If you do renew in time, all of your current settings and configurations stay as they are, and you are billed for another year of service. There's nothing else for you to do, other than continue using your Olympus smartphone app.
If you don't renew in time, you'll need to purchase another subscription, set up a new smartphone and get it connected to the server again. This takes a little more than five minutes.
Steps to Renew Your Olympus Subscription
Because this is a two-party process, involving both the Customer and the Reseller, we're going to show both sides of what happens. Be on the lookout for the ">>" tags to indicate who the instructions are for in each section.
You'll receive an email from the Olympus Dictation Delivery Service, subject line "License renewal notification [C-103]." This is the first notification that your license needs to be renewed, and it comes to you 60 days before expiration. You'll get two more along the way (if you don't renew immediately).
You will also receive an email indicating that your customer's account is 60 days away from expiration.
In order to renew, go to the Olympus Dictation Portal here. You'll need to log in using the credentials you set during the "Creating An Account" process, as outlined here.
If you've forgotten your password, use the "Forgot Password" link. If you've forgotten your user name, contact your reseller.
If your customer has forgotten their user name, log in to the Olympus Dictation Portal and find their account. Check the box next to their account name and click the [View] button. The customer needs their "Login User Name."
Once you have logged in to the Olympus Dictation Portal, find the smartphone(s) you want to renew and check the box on the far left (as indicated below). For many of you, there will only be one listed. Find the Dashboard and click "Renew License(s)." Once the section expands, click the Flag button.
You have now completed your part. Your reseller will get an email letting them know that you're ready to renew.
Once your customer has flagged their license for renewal, you'll be on standby until thirty (30) days before it expires. You cannot issue a renewal before then. When day 30 hits, you'll receive an email to let you know how many licenses they want renewed.
At that time, log in to the Olympus Dictation Portal and find your customer's account. Check the box next to their account name. Then click the Issue License button. If you have enough licenses, select "Standard" from the drop-down menu, and fill out the rest of the fields accordingly. Then click OK.

Using Dragon® Medical Speech Recognition for your Electronic Health Record
Dragon® Medical Practice Edition 2 will increase the speed and accuracy of patient documentation, create more comprehensive patient notes and dramatically reduce transcription costs. If you are using this software, be sure you are fully up-to-date, as that can affect your ability to dictate into your EHR.
The tips we give in this article may apply to any EHR that can use Dragon® Medical, including:
- Practice Fusion
Your electronic medical record may be locally installed or hosted on a server and deployed over a remote connection. The deployment method will also factor in to how Dragon Medical voice recognition works, which we will explore in a moment.
Dragon Medical Only
If you are using Dragon Medical in an EHR/EMR, you are going to need some flavor of the Dragon Medical software. Non-medical Dragon editions are not intended for use in healthcare, and likely will not provide any voice dictation functionality in your electronic record.
How Dragon Works With Your EHR
For Dragon Speech Recognition to be able to print your words inside of your medical record, it needs to be compatible with the edit controls of the text field you are using. Any field in your EHR that you can type text into has these edit controls. They essentially encode the field for text input.
When Dragon Medical is compatible with the edit controls of a text field in your EMR, and your deployment method doesn't interrupt this compatibility, you should have speech-to-text functionality there. If the edit controls are incompatible, you won't be able to dictate.
Using Dragon Templates
Instead of dictating full reports into your EHR, you should consider creating templates. These pre-configured reports can be pasted right into your EHR, and then filled in with the specific patient information. Fillable fields can be added, which allow you to move between them with a voice command… or the press of a button.
Dragon speech-to-text has all sorts of voice commands that allow you to control it and other applications. You can even control your computer, to some extent. If you have a programmable microphone, like the Nuance® PowerMic, you can assign some of these commands to buttons.
Imagine, instead of using voice commands or your keyboard and mouse, you can program your microphone buttons to:
- Turn the Dragon microphone on/off
- Call up one of your templates
- Move between fields in your report or your EHR
- Press hotkeys that your EHR uses
- Open the Dictation Box
- Transfer text from the Dictation Box to your EHR
- Open a program
- Train words, add words, add commands, and more

Using Dragon Professional and Olympus for Background Voice Recognition
Changing the Way You Think About Dictation
If you've been using the Olympus Dictation Management System thus far, you know how effective it is at managing your dictations, supporting record and playback functions, and powering your delivery to a transcriptionist.
Your transcriptionist is certainly aware of the magnificent abilities of the software. With a foot pedal in place, she can easily listen to files and type up documents. And the software helps her receive, manage, and file dictations effectively.
What neither of you may be aware of is that you are doing it the hard way. Allow me to explain.
The Dragon speech recognition system, available in a number of editions, can be leveraged in such a way that:
- You create dictations on your handheld device
- Upload them to your computer automatically
- Have them run through speech recognition
- Deliver them to your transcriptionist(s) for final correction and document production
Instead of having your transcriptionist type out an entire dictation, she only has to edit the document produced by Dragon, and then put that in its final form.
In this post, we're going to discuss some of the ways that you can successfully integrate speech recognition into your dictation-transcription workflow.
How Does It Work?
In most iterations of the integrated speech recognition/dictation set up, the author creates dictations on a DS-7000. Then they dock the device; the Olympus software opens; and their dictations are downloaded. This workflow is also supported using the Olympus Dictation App, although files will be delivered via email, rather than downloaded.
When properly configured, the new dictation files hop into a queue, where Dragon then processes them. When the speech recognition step is finished, each of the audio files has a document attached to it—containing the auto-transcribed results.
From there, either the author or transcriptionist opens the correction window, makes any necessary edits, and then exports the document in its final form. Any corrections made to misrecognitions by Dragon are integrated into the profile when adaptation is run.
This is known as background voice recognition.
ODMS Settings and Dragon
Here are a few things to remember when using ODMS and Dragon together –
- Be sure that the DS-7000 recorder associated with a particular author has the correct author name assigned to the device.
- Associate the right author name with the right Dragon profile name in the voice recognition settings of ODMS. This is slightly different between the Dictation and Transcription Modules.
- If necessary, change the author name of existing dictations in ODMS by right clicking and choosing "Edit Job Data."
Using the Olympus DSS Player for Mac
Olympus® for OS X®
Today, we're going to look at the Olympus DSS Player for Mac, which is the Apple edition of the Olympus Dictation Management System (ODMS), available for Windows only.
While ODMS and DSS Player for Mac come on the same disc, be aware that they are not the same program. We will detail many of the features and functions available in DSS Player for Mac, but first, let's talk about what's missing from the Mac side (when compared with Windows).
- Lack of device configuration – Unless you sign up for DSS Player Plus (see below), you cannot configure the recorder beyond setting an Author ID and the time and date.
- No email or FTP sending or receiving
- No speech recognition support within the software
- No Finished or Backup folders
- No routing by worktype or author
- No routing by email FTP address
- No worktypes
- No transcriptionist notifications
If any of these features is a must-have, you might want to consider working on a Windows machine instead. Otherwise, let's continue with our look at the Olympus DSS Player for Mac.
How Does it Work?
Just like ODMS, DSS Player for Mac 7.5.2 includes an audio player with volume and speed control, noise cancellation, and filtering. Here are some of the other Windows-side features it shares:
- The playback display shows both time elapsed in the recording and total time.
- You still have the ability to index the file and set priority.
- The main window will show you your dictations and their metadata, including things like length of recording, file name, author name, date created, etc.
- There is a Device Manager pane, where you can see your connected digital audio recorder, and dig through the folders if you like.
- The Dictation Folder pane shows the Download Tray, with folders A-G, and a Dictation Tray, for direct recordings. You can create new folders under the Dictation Folder if needed.
- You can organize your work to show particular file formats only. Go to Display > Sort By to sort your audio files by any displayed column.
- Choose the metadata that you want to display for all dictations.
There's not a high level of sophistication available in DSS Player for Mac, but it will do quite a bit, if you're willing to pair it with other powerful workflow and speech recognition tools. Otherwise, it's a basic dictation playback and transcription solution, designed for simple functionality.
Hands-Free Dictation with Olympus Recorders
What Does Hands-Free Mean?
If you have an Olympus dictation workflow, or have been thinking about purchasing Olympus products, this article is for you. Today, we're going to talk about the hands-free capabilities of the current Olympus professional dictation solution.
Simply put, hands-free means that with just a docking station, a foot pedal, and a recorder, you can dictate without having to physically manipulate the device to do things like stop, rewind, play back, or create new files.
Naturally, you can turn on your recorder and set it on a desk, leaving your free to do whatever you need to. That is a type of hands-free scenario; but any time you needed to stop the recording, rewind, or make a new dictation, you'd have to use your hands again.
This particular setup is more about giving you control. If you want to start a new recording, you press a foot pedal button. If you need to check on something you just said, the foot pedal is there to allow you to rewind and play the file, without having to pick up the recorder—or even look at it.
What hands-free recording offers is a threefold benefit:
- No need to touch the recorder
- No need to view the recorder
- Recorder control is simplified
If you work in a field where you use your hands a lot, a hands-free setup could allow you to easily record while you are in the middle of a task, rather than stopping to pick up the device and start a recording. This helps you keep your train of thought, and you get to keep working while you're making the dictation.
Sometimes this is a nice convenience that adds to productivity, and sometimes it's a necessity. If you work in a profession that deals with industrial materials, chemicals, organic matter, or bodily fluids, putting your hands on a recorder could very well be contraindicated. And whether your profession falls into those categories or not, your job may not enable you to stop what you're doing to make a recording.
Just as you may not be able to stop working with your hands to make a dictation, you might also need to keep your eyes focused on something other than the recorder. Professionals in many fields may need to dictate what they can see, e.g., during a medical examination. With the Olympus hands-free setup, you're able to create new recordings, listen back to them, and make edits—all without needing to look at the recorder.
In general, when making a dictation, there isn't that many functions that you need to perform. Most people will want to be able to start and stop recording, rewind and play back the file, and then create a new dictation. With Olympus hands-free recording, you have the option to configure your foot pedal to perform these (and other functions). Once you have the foot pedal buttons set, you can perform whatever functions you assigned to them.
Instead of finding a specific function by looking at the recorder, you know exactly where it is on the foot pedal. This allows you to work without having to devote attention to operating your device.

What's New in Dragon® Professional Individual v15
Nuance recently released the newest edition of Dragon Professional Individual. In conjunction with Dragon Professional Individual v15, Nuance released Dragon Legal Individual v15. All of the information in this article applies to the legal edition as well—unless otherwise specified. In general, the two editions differ only on the matters of legal case citation, formatting, and vocabulary terms.
Perhaps the biggest improvement made to Dragon Professional Individual v15 is the new speech engine. Dragon’s speech engine helps determine its capacity to accurately recognize your words and transcribe them into documents, emails, web searches, and more.
Nuance designed their Dragon speech engine to work off of the principal of deep learning, which relates to artificial intelligence. What that means for you is that Dragon Professional Individual v15 will recognize your speech with higher fidelity. You should expect to see up to a 15% improvement in accuracy compared to version 14.
Not only is this new engine higher-performance, but it also has some new capabilities. Because of its dependence on deep learning, the new speech engine will adapt to your voice in three unique ways:
If you have an accent, the new engine detects your speech inflections while you are talking. This allows Dragon to make changes on-the-fly and improve your accuracy.
The same goes for background noise. If there are other people talking around you, the new engine can take that into account. In order for this feature to work properly, the volume of the conversations, or other noises, shouldn’t be excessive.
Dragon Professional Individual v15 now supports WordPerfect X8 and Skype for Business. And, in case you missed it in v14, Nuance announced support for Office 2016 and Office 365. Naturally, that applies to version 15 as well.
There are some valuable revisions to format recognition in this new release. Nuance focused on some of the irregular elements you need to dictate, including numbers, email addresses, hyperlinks, and place names. Your accuracy should improve when dictating these items.

An Overview of Dragon® Professional Individual for Mac® 6
A New Name
If you are familiar with Dragon for Mac speech recognition applications thus far, you may notice that Nuance has changed the name of their non-medical speech recognition solution—again. We had Dragon® Dictate for Mac v4, Dragon® for Mac v5, and now Dragon® Professional Individual for Mac v6.
This change matches the naming conventions in place on the Windows-side of things, e.g., Dragon Professional Individual 15. While the Mac and Windows applications still differ in a number of ways, there are some exciting improvements included in this latest version of Dragon for OS X.
If you have an older Mac, you'll want to verify that it meets these standards. When a computer does not meet the minimum operating requirements for Dragon, you will notice slower performance from the application. That doesn't necessarily mean that it won't run, though that is also a possibility. You may not even be able to install DPI for Mac v6 if your operating system isn't up to date.
What Did They Change?
We're going to review some of the changes made in DPI for Mac v6, though this isn't an exhaustive list. First, we'll look at some of the smaller updates that are included; then, we'll expound on two of the biggest areas of advancement.
A New Speech Engine
Perhaps the biggest improvement made to Dragon Professional Individual for Mac v6 is the inclusion of a new speech engine. Nuance has redesigned their Mac speech engine to work off of the principal of deep learning, which relates to artificial intelligence research. What that means for you is that DPI for Mac v6 will recognize your speech even more quickly and accurately. You should expect to see a 15% improvement in accuracy compared to version five.
You now have the ability to transcribe in Batch Mode, a huge addition to your transcription functionality. Let's break that down. Previously, when you needed to transcribe files, you would run them through one at a time. Now you can select all of the files you need to have transcribed, and then initiate the process. They'll be transcribed in sequence, until your list of recordings is complete.
Other changes to the software include:
- Supported Applications – you can now dictate into more applications such as Apple Pages, Keynote, Numbers and Microsoft Word 2016
- Commands - One of the most helpful features Nuance added is an automatic backup of your custom commands and vocabulary.
- Dragon Help - You now have a larger and more advanced set of online resources to consult when learning how to use Dragon Professional Individual for Mac v6.
Dragon® for Mac® Medical 5 vs. Dragon® Dictate Medical for Mac® 4
If you use Dragon® Dictate Medical for Mac® v3 or v4 for your healthcare documentation, you already know how much easier it is to fill out a patient encounter using your voice. Now, you have the opportunity to upgrade to a new version of Dragon with a new name: Dragon for Mac Medical v5.
If you already have Dragon for Mac Medical v5, you'll want to get the latest update released since you installed it. Any bug fixes, improvements, and adjustments that Nuance has made to the application will be incorporated into Dragon, and you'll be fully supported on the latest Mac OS operating system.
Dragon for Mac Medical v5 cannot be installed on a tablet or a phone. It requires one of these three Mac OS X versions:
- Mavericks OS X 10.9
- Yosemite OS X 10.10
- El Capitan OS X 10.11
If you have El Capitan, which is the latest version of the Mac OS, you need to be running at least version 5.0.1 of Dragon for Mac Medical. The chances are decent that you don't have any updates installed, so be sure to check.
- It is 15% more accurate than v4, and 35% more accurate than v3. That's a pretty significant leap—roughly a 1-in-7 improvement.
- They've added new terms to the medical vocabularies, which represent 90+ healthcare disciplines. Note: there was the same number of vocabularies in the previous version.
- They removed the American – Inland Northern accent and added American – Teens.
- There is increased speech recognition accuracy using the built-in microphone. If you prefer this method of dictation, it just got a little more viable.
- Version 5 provides better speech recognition accuracy for these accents: Canadian, SE Asian, Indian, and Australian English.
- You can now use Microsoft Word to transcribe recordings. This means your output can be set to either .DOC or .RTF, and you can edit documents without having to cut and paste. This is a definite improvement over using the TextEdit application in version 4.
- Version 5 supports both Microsoft Word 2016 and Word 2011, whereas version 4 only supported the latter. If you have a newer computer, or like to stay up-to-date, this is an important factor for your documentation environment.
- You can now install from either a download or a USB stick. That means that if you don't have a DVD drive, you aren't limited to just downloading the application. And if you do, USB is definitely faster than using a disc.
- You can configure v5 to start when you boot your computer, and it's able to stay open until you reboot. This helps incorporate Dragon for Mac Medical v5 into your workflow even more seamlessly.
- There is no more Express Editor. You'll now use the DragonPad if you need to dictate into a field that doesn't support full-text control.
- Version 5 incorporates some big changes to the Status Window and the Menu Bar. This makes it easier to find things like the Correction Menu, to change profiles, or to configure your Dragon settings.
- The transcription process has been reworked, with more user guidance and the same remodel that other parts of the application have received. See Transcription Mode for more.
- Text macros have been changed to auto-texts, and are generally easier to create, organize, and use. See Text Macros for more.
- You have the ability to use voice commands to do what Nuance calls "menu walking." This means that you can access any menu functions by speaking.

Microphone Tips in Dragon® Medical Practice Edition 2
What Is the Software Microphone?
The Dragon® Medical software microphone is the entry point for all of the live dictation you need to perform—whether you’re a physician, nurse, radiologist, surgeon, dentist, or specialist. You can find it on the far left side of the DragonBar, which is the main interface module in the application.
In spite of what you might think, just because you’re speaking into a headset or a handheld microphone doesn't mean that Dragon Medical is listening to you. In addition to having the application running and an input device connected, you also need to have the software microphone activated. This alerts the program to start listening to everything that comes through your physical microphone.
Why Is the Software Microphone Important?
The software microphone is your primary feedback mechanism for knowing whether or not Dragon® Medical Practice Edition 2 is listening to you. It’s highly recommended to get into the habit of watching the indicator icon, to see whether or not the application is ready to take your voice input. Not only does this help you not waste your time saying things that the program can’t hear, it keeps Dragon Medical from hearing things it doesn't need to.
What does that mean, you ask? Well, in order for the speech recognition engine to be able to preserve a pristine picture of your voice—and in order to avoid transcribing side conversations with your medical assistant into you document—it’s important that you only have the software microphone on when you, personally, are ready to dictate. This will help protect both your profile recognition accuracy and your documentation.
Best Practices for Using the Software Microphone
In general, there are a few things you want to do when using the software microphone in Dragon Medical Practice Edition 2:
- Minimize any background noise where you are dictating. As we noted above, not only can this produce poor recognition results, but it can also eventually spoil your voice profile.
- Make sure that the software microphone is only on when you’re dictating or giving a command. It should otherwise remain off. You don’t want accidental dictation landing in the wrong field of your patient encounter. You can go back and fix it (if you catch the mistake), but we know you want it right the first time.
- Make sure that your physical microphone and the source you have currently selected in Dragon Medical match. If they do not, you are likely to get poor speech recognition results—and you run the risk of ruining the profile itself.
- Do a periodic audio check. From the DragonBar, click Audio > Check microphone. This will ensure that the volume you are speaking at, and the distance you place the microphone at, are properly accounted for.
If you've got lots of red in the volume indicator, this is the recommended solution.
- Don’t let other people speak into your profile. Dragon Medical thinks your voice sounds like you—and you’ll want to keep it that way. Doing otherwise will effectively ruin your profile. One exception is during the audio and quality checks: you may use a stand-in for these without risk of confusing the application.

You may already know about using spoken and written forms to customize how your words display. But there’s another method for manipulating how Dragon Medical Practice Edition 2 transcribes your words.
In Dragon Medical, each vocabulary word has word properties. Using these properties allows you to configure words to appear a specific way—whether related to spacing, lines and paragraphs, capitalization, or context.
Before we go any further, also note that we’re working with Dragon Medical Practice Edition 2 for Windows. The layout is only slightly different for the Mac edition of Dragon Medical, and the functionality is almost identical, so Mac users should be able to benefit from this information as well.
If you have an earlier version of Dragon Medical, such as Dragon Medical Practice Edition 1 or Dragon Medical 10, you will still have access to word properties. But, there may be some particular features that are only available in the latest version, Dragon Medical Practice Edition 2.
What Are Word Properties?
In Dragon Medical, word properties allow you to configure how many spaces go around a particular word, determine what kind of capitalization the surrounding words get, insert line and paragraph breaks, and more. You have the flexibility to set up a very specific set of display properties for any given word. You can even have the word show up completely differently when numbers are involved.
We know doctors and other healthcare professionals often need their text to look a certain way, whether due to formatting restrictions or workflow requirements, for ease of use, or simply because of a personal preference. In conjunction with the written form of the word, word properties provide the functionality you need to document patient encounters, send emails, and write referrals with the appropriate formatting.
Here are some cases where word properties would be useful:
- I need a word to display differently if it comes after numbers.
- When I dictate this word, it should create a new paragraph automatically.
- I want the cursor to tab when I say this word.
- The word after this one should always be capitalized.
- I want a new line after this word.
Keep in mind that for some of these cases, there are voice commands that can be used instead. What we’re going to discuss relates to the automation of specific formatting related to a word—so that you never have to use an extra command when you dictate that word.

Profiles And/Or Commands: If you have a profile from version 4 that you want to use in version 5, it should show up in your profile list automatically. You will need to run a brief upgrade on these profiles, which Dragon for Mac Medical will prompt you to do so when you try to open them after installation.
If you don't want to use your previous profile, but you have some custom commands that you want to keep, you can import them.
Now, let's look at the specifications your Apple® computer needs to meet in order for this version of Dragon to run successfully.

EHRs: Dragon for Mac Medical v5 has full text control in MacPractice v5 notes fields. This means that you should be able to do everything Dragon Speech Recognition can normally do in a text field. In other EHRs, Dragon for Mac Medical will still transcribe your speech, but you may have limited functionality for selection, correction, etc.

New Dragon Professional Individual and Dragon Professional Group – Which version is best for you
Nuance® Dragon® Professional is the newest version of Dragon® NaturallySpeaking Professional, Nuance is reorganizing their speech recognition software to offer functions specific either to the individual or the group (enterprise version).
Essentially, that means that some of the features you remember from Dragon NaturallySpeaking Professional 13 were split between two different markets, with a few updates and a newly integrated feature.
Dragon NaturallySpeaking Version 13 was the last time you'll see Dragon "NaturallySpeaking" Professional. It's officially called Dragon Professional now—with the suffix of either "Individual" or "Group." Instead of "DNS Pro," you may see people refer to the application as "Dragon NaturallySpeaking Professional Individual" or "Dragon NaturallySpeaking Professional Group," signifying "Individual" or "Group" respectively.
Of special note, Dragon Professional Individual 14 is supported on Windows 7, 8.1, and 10. This is the first US offering of Dragon that has Nuance's blessing for Microsoft's latest operating system. Dragon NaturallySpeaking Professional Individual also supports Office 2010 and 2013 – so you're covered through the latest versions of Microsoft Office applications (Word, Excel, and Outlook being the most common).
Upgrading Dragon Professional
If you have one of these versions/editions of Dragon NaturallySpeaking, you can purchase an upgrade license for Dragon NaturallySpeaking Professional Individual 14 and save a little money:

- DNS 13 Pro or Premium
If you install Dragon Professional Individual 14 over any of the above applications, program will recognize the previous version. You only need the new serial/license number. However if you uninstall your previous version of Dragon NaturallySpeaking, Dragon NaturallySpeaking Professional Individual 14 will ask you for your new serial number as well as the serial number from one of the above versions. Having the old installation disc is not enough.
What Does "Group" Mean?
Dragon Professional Individual 14, in contrast with Dragon Professional Group 13.5, is an application tailored for one to several users in an environment that doesn't have IT support or enterprise implementation. The multi-user features that you may not have used in Dragon NaturallySpeaking Pro 13 are now moved over to Dragon NaturallySpeaking Professional Group.

If you do work on your smartphone, this might improve your experience significantly.
New Dragon for Mac V5
Nuance has released their newest speech-to-text application for OS X: Dragon® for Mac 5. There are several changed to this latest version. The first thing you may notice is there is no "Dictate" in the name any more. More importantly, thotugh, let's see what updates have been made to the system requirements.
You can run Dragon for Mac 5 on OS X Mavericks (10.9), Yosemite (10.10), and El Capitan (10.11). This is the first significant advancement we see in v5. Take special note that you must have version 5.0.1 (at least) for El Capitan support.
These features of Dragon for Mac v5 are exactly the same as v4, with one exception:
- Audio formats for transcription
- Vocabulary Editor
- Programmable hotkeys
- Commands Manager – Text macros can be viewed here, but not added. Other commands are the same as before.
The Big Changes
Here's where Dragon for Mac v5 really diverges from v4, with improvements to accuracy, support for more applications.
- "15% more out-of-the-box accuracy than Dictate 4"
- Full support for Word 2016
- Better compatibility with the internal microphone
- Voice commands to select from any Mac menu

In this new Dragon release for the Mac platform, the Status Window now allows you to access more functions with less navigation. This redesign centers around the expand/contract principle—which Nuance also incorporated into their DragonBar refresh for Dragon Professional v13.
Instead of clicking on the Dragon icon in the Dock, and then navigating through the menu bar at the top of your monitor, Dragon for Mac now has a persistent menu icon. Open Dragon for Mac v5 and look at the top right of your screen for a black flame symbol. From this icon, whenever the program is open, you can access menu items without having to switch back to the application.
Some of the most helpful things you can do from the new menu icon include:
- Switch profiles and sources on-the-fly
- Enter transcription mode
- Access "Voice Training," "Vocabulary Training," and "Microphone Setup" (under "Improve Recognition")
Starting with Dragon for Mac v5, text macros have a new name and a simpler process for creation.
These commands are now called auto-texts, and they've been separated out within the Commands Manager to make them easier to identify. It used to be the case that you had to look through the commands by application/context to find your text macros. Now, "Auto-text" has its own category.
There's a minor issue, though. If you select the "Auto-text" category in the Commands Manager window, you'll be able to view all of your auto-texts—but that's it. You cannot add new ones from here.
If You're Still Waiting to Upgrade
Remember, Dragon for Mac v5 is the only version of Dragon Speech Recognition that is officially supported on El Capitan. Aside from that, the UI redesigns make it easy to figure out what you're doing, even if you don't consider yourself tech savvy. And don't forget about the new transcription setup. For the first time, Dragon Speech Recognition now produces documents in a folder, rather than just pasting your transcription into a text window. That folder could be on your local machine, or on the network—which makes Dragon for Mac v5 even more attractive to anyone who works with dictation and transcription regularly.
Activate Dragon Medical Practice Edition 2’s Hidden Mode with a Nuance PowerMic II
One of several features unique to Dragon Medical Practice Edition 2 is the Hidden Dictation Box, commonly referred to as Hidden Mode. The Hidden Dictation Box allows users to dictate into Dragon Medical Practice Edition 2 while their cursor is active in another application. This is a must-have for Radiologists and any other clinician needing to focus their attention elsewhere while dictating.
This guide will walk you through the process of programming a Nuance PowerMic II:
- Active the Hidden Dictation Box feature within Dragon Medical Practice Edition 2.
- Transfer your text into the application of your choice with the press of a button.
Prerequisite: You must have a user profile created within Dragon Medical Practice Edition 2, with the PowerMic II selected as a dictation source.
Programming Your Microphone
- Open Dragon Medical, and go to Tools > Options. You will then be presented with the Options dialog window. Select the [PowerMic] tab.
- You can program any of the buttons to activate the Hidden Dictation Box and Transfer Text functions, for this guide, I will choose the Custom Left and Custom Right buttons. Within the [PowerMic] tab, select the drop-down menu next to Custom Left, and then choose Recording Modes > Hidden Mode on/off.
- Now select the drop-down menu next to Custom Right and choose Dictation > Transfer Text.
- Select [OK] to save your changes. You've just programmed your Nuance PowerMic II to activate Hidden Mode and Transfer Text.
Let's Test It
5. While working in the application of your choice, activate Hidden Mode by pressing the Custom Left Button. Once the Hidden Dictation Box has been activated, two things will happen.
First, the mode icon on the DragonBar will turn into an outlined square as a visual indication that you are working in Hidden Mode.
And once you begin dictating, lines will appear within the outlined square. These lines let you know there's dictation waiting to be transferred.
Second, you'll see a faint, transparent Dictation Box window appear. This is the actual Dictation Box, and you can adjust its transparency by going to Tools > Options > Dictation Box. The default is set to 50%, but you can set it to 0% to keep it completely hidden.
Begin dictation then select the Custom Right button to transfer the dictation into the application of your choice.
Congratulations, you've just programmed your Nuance PowerMic II using Dragon Medical Practice Edition 2. You can now automatically launch Hidden Mode and transfer your text with the press of two buttons.
How to Use Hidden Mode in Nuance® Dragon® Medical Practice Edition 2
What Is Hidden Mode?
For users of Dragon Medical Practice Edition 2, Hidden Mode provides two functions you wouldn't normally have access to:
- If your electronic medical record requires the use of the Dictation Box, Hidden Mode allows you to work almost as seamlessly as you would were you dictating directly into the patient chart.
- This mode also enables you to review other websites or applications, dictating notes without having to click the Dictation Box, and without it getting in the way of your research.
Before we go further with Hidden Mode, let's talk a little about the Dictation Box—because you may have never used it before.
What Is the Dictation Box?
In case you didn't know, the Dictation Box is the tool that Dragon Medical Practice Edition 2 uses to interface with the myriad EMRs available to clinicians. While it does serve other purposes as well, it is primarily a platform for integration.
You can find the Dictation Box by going to the Dragon Bar and clicking on Tools > Dictation Box. Or, you can simply use the verbal command, "Open Dictation Box."
What Does It Mean to "Anchor the Dictation Box"?
If you go into the settings menu of the Dragon Bar, you'll see a curious feature on the Dictation Box tab. Click Tools > Options > Dictation Box and find the checkbox at the top that says, "Anchor Dictation Box to application."
This is not Hidden Mode. Rather, anchoring the Dictation Box tethers it to the active application at the time it is opened.
For instance, open Notepad and the Dictation Box (anchored), and then start up a web browser. As usual, Dragon will change your speech into text and place it in the Dictation Box. When you use the "Transfer Text" command, your dictation will then be pasted into Notepad, regardless of where you click your mouse or which application is active. Then, the Dictation Box closes.
How Do I Use Hidden Mode?
Hidden Mode is slightly different. Go back to the Dictation Box tab in the Options menu. Jump down to the section labeled "Hidden Dictation Box behavior."
Here are the recommended boxes to check:
- (Required) Open Dictation Box as hidden
- Allow user commands when hidden
- Allow voice dictation commands when hidden
- Leave Dictation Box open after text is transferred
Also, leave "Dictation Box visibility" at 50% to start with—you can always change the transparency to suit your tastes.
With the above configuration, here's what will happen when you open the Dictation Box:
- The Dictation Box will start up in a translucent state.
- Any dictation that you make will end up in the Dictation Box, no matter where your mouse is pointed.
- In order to move a dictation to a specific field or application, click wherever you need the text to go and say, "Transfer text."
- After you transfer your Dragon-recognized text, the Dictation Box remains open.
- Continue with your patient note. There's no need to click back to the Dictation Box—Dragon Medical will put your text there automatically.
How Does That Help Me?
First of all, Hidden Mode powers you to do research and make notes without having to worry about whether your mouse is clicked into the Dictation Box. It also allows you to make the Dictation Box as visible or invisible as you need it to be.
Second, and more importantly, Hidden Mode enables you to work effectively within an EMR that doesn't play well with Dragon Medical Practice Edition 2. In essence, opening the Dictation Box as hidden allows you to dictate parts of the patient chart as needed, without having to move back and forth between Dragon and your EMR.
With Hidden Mode, you only need to dictate, click into the appropriate EMR field, and say, "Transfer text."

But before we dive head-first into an exploration of how the application works to secure your dictations, let's talk a little bit about HIPAA itself, as well as what it requires when it comes to Protected Health Information, commonly referred to as PHI.
HIPAA Regulates PHI Access
HIPAA stands for the Health Insurance Portability and Accountability Act. It was created, in part, to protect people from having their personal health information released to or viewed by anyone who shouldn't have access to it.
HIPAA, originally put in place in 1996, was fully implemented by 2003—although it wasn't until the recent HITECH Act that EHRs became subject to HIPAA. These acts essentially work together to prevent personally identifiable patient data from being shared without the patient's consent.
In general, though not exclusively, the patient decides who should access their PHI—healthcare providers and insurance companies, notwithstanding. That's why they have you sign a Notice of Privacy Practices form when you visit the doctor's office. You have to acknowledge you understand how your PHI may be shared.
The parts of HIPAA that you really need to concern yourself with, for the purposes of Dragon Medical, are what's known as the Privacy Rule and the Security Rule. One says that you have to protect patient data; the other tells you what standards you need to meet to protect it.
How Does Dragon Medical Software Protect PHI?
So, what does this mean? What does Dragon Medical Practice Edition 2 have to protect, specifically? Is the program considered HIPAA compliant?
Let me address that last question first.
HIPPA requires that Covered Entities and Business Associates handle PHI properly. Even if an application helps them do this, it is—in no way—a substitute for or a solution to the question of compliance.
That being said, Dragon Medical Practice Edition 2 does support HIPAA compliance. That means that the engineers at Nuance thought about healthcare providers when they were building the application, and they included features that they knew would make it possible to secure PHI while documenting patient encounters.
Now, there's really no practical way for Dragon Medical Software to know whether a clinician happens to be dictating what would be considered individually identifiable health information, or just a friendly letter to a colleague. That means that the software has to assume that everything being dictated is sensitive and needs to manage accordingly.
Tracing Speech and Text Through Dragon Medical
In order for us to understand how Dragon Medical Practice Edition 2 supports HIPAA compliance, let's follow the voice and text data as it travels throughout the program. Keep in mind that the only part of your workflow that the software can affect is the part that it's involved in.
What follows represents a standard workflow for clinicians dictating into an EMR.
When you begin to dictate into any program, Dragon Medical creates audio and text files. These are known as 'session files,' and they are temporary.
If you make corrections, they get put into .enwv files. These are used for optimization, and are encrypted before being saved to the profile.
When the session is over, your temporary Stage One files get put into a new .DRA file. This is then encrypted and saved to the local profile, or the local copy of the roaming profile.
During optimization, Dragon Medical Practice Edition 2 will use the still-encrypted .DRA and .enwv files to try and improve your local profile. Roaming profiles may transfer these files to the network roaming directory when synchronization occurs, where they can then provide data for optimization.
All of the above encrypted files must be opened by Dragon Medical. The DragonPad can't access them, because it's a feature present in non-medical versions of the software.
Potentially Unprotected Data
Now, let's talk about where you might need to shore up your defenses with regard to Dragon Medical and PHI.
- First, have a look at Tools > Administrative Settings > Miscellaneous. 'Encrypt patient health information' should be checked, but you'll want to verify this.
- Next up, take note that audio files you save on purpose—when dictating into a word processing application—aren't encrypted. You'll know whether or not this is happening by navigating to Tools > Options > Data > 'Save recorded dictation with document.' If the setting is 'Never,' you're in the clear. If it's 'Ask Me' or 'Always,' you're either being prompted to save unencrypted data, or you're saving it automatically. Make sure your PHI is protected.
- If you want to stop creating .DRA files (the encrypted kind described in Stage Three), go to Tools > Options > Data again and check 'Conserve disk space required by user profiles (for portability).'
- Finally, make sure to uncheck 'Store corrections in archive' in the same Tools > Options > Data window we've been working in. Otherwise, you could be creating unencrypted .nwv files when you make corrections. We don't want that. This last change may somewhat limit your ability to optimize your Dragon Medical profile. If this is a concern, we recommend running profile optimization more often.

Please remember that HIPAA compliance is the responsibility of Covered Entities and their Business Associates. We're just arming you with information that you can use to help meet your Privacy and Security Rule obligations as you operate Dragon Medical Practice Edition 2. And this information only applies to that particular version.

While the command updates apply to front-end voice recognition only, templates and fields apply to both front and back end.
Front-end (aka real time) speech recognition in ODMS 6.3.1 now supports additional Dragon commands including the creation of custom commands, switching dragon modes, moving between variable voice fields, and microphone sleep/on toggle.
Some extra flexibility has been given in terms of how you choose voice recognition templates to use. When setting up your options, you no longer have to specify 'Author' or 'Worktype.'
